2012 Application Information

1. Download & Print using either one of the provided format. One is in Microsoft Word (.doc format) and the other is in Adobe Acrobat (.pdf format). 

2. Please read carefully and give us as much detail of your background as possible in order for us to evaluate accordingly. We strongly encourage all applicants to send us a recording of your playing, because even though it is not a requirement, it will help us in making the best placements. You could attach an MP3 file to an email, or send a well-packaged CD or DVD, or the brave could even post a video on YouTube. Let us know if you do. 

3. Make sureyou indicate which week you are applying for. 

4. *IMPORTANT* Mail BOTH Part 1(page 1) and Part II (page 2) of this Application form along with your $200.00 deposit to:

Workshop Policies & Cost

Enrollment is limited for both Workshop weeks – There will be a Minimum of 40 and a Maximum of 64 participants each week.  Applications will be accepted on a “first-come, first-served” basis with priority given to previous participants until March 15^th, 2012.  This Application Form (both Part I and Part II) must be accompanied by a deposit of $200.00 which is non-refundable after June 1, 2012.  The entire balance of Tuition will be due by June 30, 2012 or a late fee of $75.00 will be added.  For cancellations before June 30, 2012, refund is 100% less deposit.  No refunds allowed after July 1, 2012.
